Complex & Revision Spine Surgery

Complex spine surgery is surgery for spine problems that need more careful planning than routine cases. This includes multilevel degenerative disease, spine fractures, spinal infections like spinal TB, spinal tumours and patients with weak bones or many health problems. Revision spine surgery is surgery done on a patient who has already had spine surgery before, when the pain has continued or come back due to a repeat disc slip, scar tissue, loose screws, a failed fusion or new problems at nearby levels.

These surgeries are more challenging because the spine may be unstable, the anatomy may have changed, and scar tissue or old implants can make the nerves harder to reach safely. During his training at TNMC & Nair Hospital, Mumbai, Dr. Tanmay Avhad managed a high volume of complex spine cases, including fractures, infections, tumours and revisions, and has published research on spinal TB and cost effective spine fixation. At SpineArc, he combines this experience with endoscopic and minimally invasive techniques, so that even complex and revision cases can often be treated through smaller cuts, with less risk and faster recovery.

Why is it performed?

Complex and revision spine surgery is performed when a spine problem affects the stability of the spine, puts pressure on the spinal cord or nerves, or has not been solved by earlier treatment or surgery. It is used for multilevel spinal stenosis and degenerative disease, spine fractures and fracture dislocations, spinal infections and TB, spinal tumours and metastatic disease, recurrent disc herniation, adjacent segment disease, failed fusion, loose or broken implants, and instability after earlier surgery. The aim is to relieve nerve pressure, stabilise and realign the spine, control infection or tumour, and reduce pain, so that the patient can regain function, walk and return to daily life. Every case is planned carefully based on the patient’s overall health, bone quality and earlier surgery details.

How is it performed?

Every complex or revision surgery starts with detailed planning. Dr. Tanmay reviews the patient’s earlier surgery notes and old scans, and uses fresh MRI, CT scans, bending X-rays and, when needed, a CT guided biopsy to understand the exact problem. Based on this, the least invasive safe approach is chosen. In revision cases like a repeat disc slip or residual stenosis, endoscopic revision surgery can often reach the problem through a fresh path that avoids the old scar tissue, through a small cut. For multilevel nerve pressure, endoscopic decompression can be done at more than one level through small cuts. For fractures, infections and tumours, the spine can be stabilised with percutaneous screws placed through small skin cuts, along with decompression of the nerves or cleaning of the infected area. In cases with loose implants, failed fusion or major instability, the old implants are revised and the fusion is redone or extended, using minimally invasive or open techniques as needed. C-arm navigation and intraoperative neuromonitoring are used throughout to protect the nerves.

Recovery Period

Recovery after complex and revision spine surgery depends on the type of problem and surgery done. After endoscopic revision or decompression, most patients walk within a day and go home in 1 to 2 days. After minimally invasive fixation or fusion, patients usually walk with support within 1 to 2 days and go home in about 3 to 5 days. Larger reconstructions, infection or tumour cases may need a longer hospital stay and a structured rehabilitation program. Light activities are usually resumed in 2 to 4 weeks, while full recovery and bone healing can take 3 to 6 months. Patients with spinal TB or cancer continue their medical treatment alongside recovery, with regular follow up visits and scans.

GOT QUESTIONS?

Frequently Asked Questions

Everything you need to know about complex and revision spine surgery in Mumbai, answered simply by Dr. Tanmay Avhad.

What is complex spine surgery?

It is surgery for difficult spine problems like multilevel disease, fractures, infections, tumours or patients with many health issues, which need detailed planning and advanced techniques.

What is revision spine surgery?

It is a surgery done when problems continue or return after an earlier spine surgery, like a repeat disc slip, scar tissue, loose screws or a failed fusion.

Is revision spine surgery riskier than the first surgery?

Revision surgery can be more challenging due to scar tissue and changed anatomy. Careful planning, endoscopic techniques and neuromonitoring help reduce risks in suitable patients.

Can complex spine surgery be done with small cuts?

In many cases, yes. Endoscopic decompression and percutaneous screw fixation can treat multilevel disease, fractures, infections and some revision cases through small cuts. Larger problems may still need open surgery.

Will my old screws and rods need to be removed?

Not always. Old implants are removed or changed only if they are loose, broken, infected or blocking the treatment. Many revision surgeries can be done without touching the old implants.
